Section 1: Participation Acknowledgement & Risks

Participant must check all boxes

Understanding the Activity: I acknowledge and represent that I understand the nature of the Activity and that I am qualified, in good health, and in proper physical condition to participate.

Safety Warranty: I agree and warrant that if I believe conditions to be unsafe, I will immediately discontinue participation.

Assumption of Risk: I fully understand that this activity involves risks of serious bodily injury, including permanent disability, paralysis, and death ("Risks").

Acceptance of Responsibility: I accept and assume all such risks and all responsibility for losses, costs, and damages incurred as a result of participation.

Section 2: Release and Indemnity Agreement

Release of Liability: I hereby release and discharge the sanctioning organization, administrators, volunteers, and owners of the premises from all liability or claims caused by the negligence of the "Releasees".

Indemnification: I agree that if a claim is made on my behalf, I will indemnify and hold harmless the Releasees from any litigation expenses or attorney fees.

Full Understanding: I acknowledge I am 18 or older (or a legal guardian) and understand I have given up substantial rights by signing this agreement.
